It is February 2026, and many doctors are feeling an adrenaline rush as it is time for them to face the MRCPI OBG Part 2 OSCE. This exam is the final practical part, serving as a gateway to Membership of the Royal College of Physicians of Ireland. This exam differs from the written exam, where you battle against a computer screen that asks you 100 Single-Best-Answer questions. The OSCE (Objective Structured Clinical Examination) assesses your clinical practical skills, requiring you to act as a senior OBGYN consultant.

The Anatomy of MRCPI OBG Part 2 OSCE

Exam Structure

The MRCPI OBG Part 2 OSCE/ Clinical examination is a practical exam with seven ten-minute stations and at least one rest station. The rest station is a short break and an opportunity to prepare for the next station.

At the clinical exam, you will see one long clinical case in either obstetrics or gynaecology. You have 25 minutes fully observed with two examiners to take a history and examine the patient/ role player before discussing the case with the examiners.

• The clinical component is simulated; this component will not be based in a hospital and will not have a patient interaction.
• This clinical exam has a duration of 25 minutes, in which you will be examined by a pair of examiners on a simulated clinical case using model equipment in either obstetrics or gynaecology. You may also be expected to interact with a roleplayer.

The MRCPI OBG Part 2 OSCE examinations are held across international hubs. The details of the current exam are:

Early Pregnancy:

• Ectopic Pregnancy: You will have to counsel a patient on the management options (Methotrexate vs. Surgery vs. Expectant) and explain the safety netting for each.
• Misarriage: You’ll have to break the bad news if your patient experiences a miscarriage, which tests your empathy and communication skills.

Antenatal Care:

• High-Risk Counselling: Explaining the implications of a high BMI, advanced maternal age, or refusal of blood products.
• Reduced Fetal Movements: You have to display your ability to know the indication for induction versus conservative monitoring.

Maternal Medicine:

• Medical Complications: You will manage Epilepsy, VTE prophylaxis, and pre-existing Diabetes or Hypertension.
• Sepsis: The examiners also evaluate your skill to recognise the Red Flags early in a pregnant patient with vague symptoms.

Labour:

• Poor Progress: Interpreting a partogram and deciding when to augment with oxytocin versus when to recommend a Section.

Delivery:

• Obstetric Emergencies: Drills are key here. Shoulder Dystocia, Breech delivery, and Cord Prolapse need to be second nature.
• Operative Birth: Counselling a patient for a forceps or ventouse delivery, including a clear explanation of risks.

Postpartum:

• PPH Management: You will have to manage a massive haemorrhage in a real-time simulation.
• Secondary Issues: Perineal wound breakdown or severe preeclampsia presenting in the puerperium.

SRH (Sexual & Reproductive Health):

• Contraception: UKMEC criteria application. Counselling a teenager or a woman with comorbidities (e.g., migraines) on the best method.
• STIs: Contact tracing discussions and managing PID.

Subfertility:

• Investigation: Interpreting a seminal Fluid analysis or HSG and explaining the results in plain English.
• Management: Explaining the differences between IUI and IVF to a couple with unexplained infertility.

Urogynaecology:

• Incontinence: Differentiate Stress and Urge Incontinence and explain Urodynamics.
• Prolapse: Counselling on conservative management (pessaries) versus surgical repair.

General Gynaecology:

• Menstrual Disorders: Candidates have to display their ability to manage Heavy Menstrual Bleeding (HMB) in a 45-year-old.
• Key Skill: Tailoring medical vs. surgical options to the patient’s lifestyle and fertility desires.

Core Surgical Skills:

• Instrument Identification & Consent: You will have to identify surgical tools or walk through the consent process for a Diagnostic Laparoscopy, ensuring the patient understands the specific risks of trocar injury or conversion to laparotomy.

Post-Operative Care:

• The Deteriorating Patient: You will manage post-op complications such as anuria, suspected pulmonary embolism, or a spiking fever on Day 1. Use the A to E approach every single time.

Oncology:

• Screening and Referrals: You have to know the “Two-Week Wait” criteria for suspected gynaecological cancers and be able to explain the significance of an abnormal cervical screening result to a worried patient.
